PATIENT REQUEST FOR MEDIATION
The Toledo Dental Society maintains a Peer Review Committee to assist
the public and the profession to amicably settle disputes involving the
appropriateness of care or the quality of care in cases that are less than one
year old, and service was provided in Ohio's Lucas, Ottawa, or Wood Counties.
The Peer Review Committee members are dentists who volunteer their time
and expertise to review such disputes. A dispute may be resolved
through mediation alone, or through a full committee hearing involving
the patient, the dentist, patient records, and a patient clinical
examination.
To begin the process, complete the
Patient Request for Mediation form. All complaints must be
submitted in writing.
